Comments:

An unannounced renewal inspection was conducted on December 7, 2021 2021 from approximately 9:30 am to 4:35 pm at the Richmond office of Bethany Christian Services of Virginia. An entrance meeting was conducted with the administrative assistant. The administrative assistant and branch director were available throughout the inspection. During this inspection, 5 adoptive home records, 1 adoptive child record, and 1 birth mother record was reviewed. Background checks for 6 additional adoptive homes were reviewed. Six staff records were reviewed and background checks for the two corporate board officers were reviewed. One employee was interviewed during the inspection. The branch director participated in the exit interview conducted on 12/7/2021 to discuss inspection findings. There were citations for violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on review of the record for Adoptive Home (AH) AH9, the licensee failed to ensure all members of the household were listed on the face sheet in the record at the time of the inspection. Findings: The homestudy in the record for AH9 documents that Other Household Member (OHM) OHMI resides in the home. The face sheet in the record at the time of the inspection for AH9 failed to list OHM1 as a household member.
