Comments:

An unannounced renewal inspection was conducted on 2/24/22 from approximately 10:00 am to 4:00 pm. The executive director and corporate compliance manager were available throughout the inspection to answer questions. The agency reported 11 children in care and 28 approved provider homes for this location of ADORE. The inspector reviewed 4 staff records, 2 children's records and 2 provider home records. Two staff were interviewed during this inspection. An exit interview was conducted on 2/24/22 with the executive director and corporate compliance manager from approximately 3:10 to 4:00 pm to discuss preliminary inspection findings. An acknowledgement form was left with the executive director. There were four violations of the Standards for Licensed Child Placing Agencies. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on review of the record for PH1, the licensee failed to ensure the applicant was notified in writing of the approval decision. Findings: PH1 was approved on 12/17/21. At the time of review, PH1's record failed to document written notification to PH1 regarding the approval decision.

Description:	Violation: Based on review of the record for Provider Home (PH) PH1, the licensee failed to ensure the pre-service training included the 22 core competencies as required by 22VAC40-131-210-A.1-22. Findings: PH1 was approved on 12/17/21. At the time of review, PH1's record documented a certificate of completion of pre-service training. The record did not document that the pre-service training included the required 22 core competencies as required by 22VAC40-131-210-A.1-22.

Description:	Violation: Based on review of background checks for Staff (S) S2, the licensee failed to ensure the sworn disclosure was completed before three years since the date of the last sworn disclosure. Findings: The most recent sworn disclosure for S2 is dated 3/25/21 and the previous sworn disclosure is dated 10/25/16, which is beyond three years.
