Comments:

An unannounced monitoring inspection was conducted on 3/16/2022 from 9:30 a.m. to 3:45 p.m. An entrance conference was conducted with the Executive Director. The agency reported a census of 22 approved foster homes and 36 foster children in placement. No new staff were hired since the last inspection. The inspector reviewed four (4) foster child and three (3) foster home records. Caseloads and office setting were inspected. An exit interview was conducted with the Executive Director and the Supervisor/Foster Parent Coordinator on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. The information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility. Upon receipt of the Violation Notice, the licensee is encouraged to develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on record review and interview, the agency failed to obtain an annual physical examination report for Foster Child 2 (FC2) addressing requirement elements. Findings: 1) A written physical examination report for FC2, dated 8/6/2021, did not address the child?s auditory acuity. 2) The findings were discussed during the exit interview and Staff 1 and 2 (S1 and S2) were interviewed. 3) S1 and S2 acknowledged the findings.
