Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 5/31/2022 from 10:45a.m. to 5:00 p.m. and 6/1/2022 from 9am to 12pm Number of children in care: 28 Number of approved provider homes: 31 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: 3. Number of provider home records reviewed: 4 and 1 closed foster home record. Number of staff records reviewed: 2 Observations by licensing inspector: positing of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection. The evidence gathered during the inspection determined (2) violations with applicable standard(s) or law. Standard #:	22VAC40-131-340-E-3
Description:	Based on the review of the records presented for Foster Child (FC)-2 and based on an interview with Agency Representatives (AR), the licensee failed to develop an individualized service plan with goals and objectives to meet the child?s needs that include specific independent living services to be provided to assist the child in meeting his goals for a youth 14 years of age or older. Findings: 1. Based on the review of the individualized service plan for FC-2, there were no independent living goals or objectives listed on the plan. 2. Based on the review of the individualized service plan for FC-2, the plan did not list goals and objectives to meet the child?s needs that include specific independent living services. 3. During the exit interview AR-1 confirmed the youth?s age of 16 and acknowledged the absence of an individualized service plan that documented goals and objectives to meet the child?s needs that include specific independent living services.
Plan of Correction:	The Executive Director will conduct a training to ensure that all staff are aware of the requirement to develop the independent living services to be provided to assist youth 14 years and older. The training will also include the independent living goals and objectives that are needed for the youth 14 and over. The Executive Director and the designee will review all treatment plans to ensure that the applicable independent living goals and objectives are documented on the plan.

Description:	Violation: Based on a review of the sworn statements provided for Foster Home (FH) FH4, Individual #1 and Foster Home (FH) FH5, Individual #2, the licensee failed to require individuals, for whom background checks are required, to fully complete the sworn statement. Findings: Individual #1 is an adult household member of FH1. Individual #1 sworn statement dated 2/22/2022 did not contain an answer to form question #5. Individual #2 is an adult household member of FH5. Individual #2 sworn statement dated 8/19/2021 did not contain an answer to form question #5. During the exit interview with AR-1 it was confirmed and acknowledged the absence of question #5 on the sworn statement documentation for FH4 individual #1 and FH5 individual #2
Plan of Correction:	The Executive Director will conduct a training to ensure that all staff are aware that all sections of the sworn statement must be addressed. The Executive Director will review all sworn statements to ensure that all areas are addressed.
