Type of inspection: Monitoring
Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 6/15/2022 11:55 a.m. to 4:00 p.m. This inspection was conducted by two Licensing Specialists. An entrance conference was conducted with the Chief Program Officer of Child and Family Services.

Number of children in care: 10
Number of approved provider homes: 25
The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Number of staff records reviewed: 1
Number of Provider Home records reviewed: 3
Number of interviews conducted with staff: 1
Background Checks reviewed: 1 staff, 10 provider homes
An exit meeting was conducted by virtual meeting on 6/16/2022 with the Chief Program Officer of Child and Family Services.

Description: Violation:
Based on record review and interview, the agency failed to ensure there were no more than 13 months between medical examinations received by the child.

Findings:
1. The case record for Child 1 (C1) contained a physical examination report documenting an examination date of 4/21/2021.
2. The case record for C1 did not contain additional medical examination reports.
3. Staff 1 (S1) was interviewed.
4. S1 reviewed C1?s case record and did not locate additional medical examination reports.
5. S1 acknowledged the findings.

Plan of Correction: CHS will ensure that no more than 13 months will pass between medical examinations received by children, and the case record for C1 will include documentation of routine and timely physical examinations for the child.

Based on record review and interview, the agency failed to arrange for Child 1 (C1) to receive routine dental care and treatment and maintain documentation regarding the arrangements for and the child?s receipt of care in the child?s file.
Findings:
1. The case record for Child 1 (C1) contained a dental examination report documenting a routine examination date of 5/3/2021.
2. The case record for C1 did not contain additional routine dental examination reports.
3. Staff 1 (S1) was interviewed.
4. S1 reviewed C1?s case record and did not locate additional routine dental examination reports.
5. S1 acknowledged the findings.

Plan of Correction: CHS will ensure that the file of C1 will reflect documentation of routine and timely dental care will be maintained for the child.
