Type of inspection: Renewal Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 6/27/2022 from 10:15a.m. to 1:00 p.m. The Licensing Inspector met with the Executive Director at the site. Number of approved provider homes: (8) Number of children in care: (8) The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: (1) and (1) discharged children?s record. Number of provider home records reviewed: (1). Number of staff records reviewed: (2) and the (4) board officers. Observations by licensing inspector: posting of the license and inspection documents was observed. An exit meeting was conducted on the date of inspection. There were no changes made to the policies or procedures during the licensing period. The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. Description:	Based on the review of the records presented for Foster Child (FC)-1 and based on an interview with Agency Representatives (AR), the licensee failed to document and arrange for the child to receive routine follow-up dental care. Findings: 1. Based on the review of the Foster Child (FC)-1?s child record, there was no documentation that a follow-up dental care appointment or treatment was received based on the previous visit appointment form dated 12/2021. 2. There was no follow-up appointment arranged and documented in the child?s file. 3 During the exit interview AR-1 confirmed and acknowledged the absence of documentation and arrangement of follow-up dental care.
