Type of inspection: Monitoring An on site inspection was conducted on 1/18/23. An exit interview was conducted on 1/18/23. The Acknowledgement of Inspection form was signed and left at the facility on the date of the inspection. Number of children in care: 3 Number of approved provider homes: 21 The licensing inspector completed a tour of the physical plant that included the office setting and conditions. Number of children?s records reviewed: 1 birth parent/child file reviewed Number of staff records reviewed: 1 Number of interviews conducted with staff: 1 entrance interview and 1 exit interview with the Social Service Program Manager Observations by licensing inspector: The license was posted along with inspection documents. Additional Comments/Discussion: 3 adoptive home records were reviewed The evidence gathered during the inspection determined a non-compliance with an applicable standard or law, and a violation was documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation will be addressed in order to return the facility to compliance and maintain future compliance with the applicable standard or law. If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview. Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected. Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector?s immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area. Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice. Description:	Violation: Based on a review of an adoptive family file (AH-3) and an interview with an agency representative (AR-1), the agency failed to obtain a search of the child and abuse neglect registry maintained by any other state in which the individual has resided in the last 5 years as required. Evidence: 1) The sworn statement for another adult household member (AHM-1) residing in the home of AH-3 dated 4/15/22 documented that this person resided in North Carolina within the last 5 years. 2) There were no child abuse and neglect registry results from North Carolina in the file for AHM-1. 3) AR-1 acknowledged in an email on 1/19/23 that a request for the child abuse and neglect registry results for AHM-1 was sent to North Carolina on 7/06/22 but that the results were never received.
