Type of inspection: Initial An initial inspection was conducted on 12/5/2023 from 10:30 a.m. to 11:30 a.m. on site. A desk review of staff records was held from 1:20 p.m. ? 2:15 p.m. and on 12/6/2023 from 12:15 to 1:15 p.m. The Acknowledgement of Inspection form was provided via e-mail. An exit was conducted on 12/8/2023. Number of interviews conducted with staff: One Observations by licensing inspector: Office settings and conditions were observed. Staff reviews: Seven The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
