Type of inspection: Monitoring An unannounced mandated inspection was completed on 12/20/2023 from 11:30am. to 5:00 p.m. The Acknowledgement of Inspection form was signed on the date of the inspection. A preliminary findings review was held on the date of the inspection. An exit findings review was completed on 01/03/2024. Description:	Violation: Based on record review, the agency failed to conduct a progress review of Child 1 (C1) every 90 days. Findings: 1) The record of C1 did not contain a written progress review report for the period ending November 12, 2023. 2) The finding was discussed during the preliminary findings review. 3) Staff 1 and Staff 2 agreed to the findings, that the progress review report could not be located in the record and therefore, was not completed for C1 for the period ending November 12, 2023. 4) Staff 2 acknowledged the findings on the acknowledgment inspection form.
