Comments:
Type of inspection: Renewal
An unannounced renewal inspection was conducted on-site 02/13/2024 from 12:00 pm to 2:30 p.m. Remote review was conducted on 02/14/2024 to review documents forwarded by the facility.
The Acknowledgement of Inspection form was signed and left at the facility on the date of the on-site inspection. A preliminary findings review was held on 02/15/2024.

The exit meeting was conducted on February 20, 2024.

The licensing inspector completed a tour of the physical plant that included the office setting and conditions.

Number of interviews conducted with staff: 1

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.
If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.
Compliance with all applicable regulations and law shall be maintained and any areas of noncompliance must be corrected.
Within 15 calendar days of your receipt of the inspection findings (inspection summary, violation notice, and supplemental information), you may request a review and discussion of these findings with the inspector's immediate supervisor. To make a request for review and discussion, you must contact the licensing supervisor at the regional licensing office that serves your geographical area.
Regardless of whether a supervisory review has been requested, the results of the inspection will be posted to the DSS public website within 5 business days of your receipt of the Inspection Summary and/ or Violation Notice.
The department's inspection findings are subject to public disclosure.

Description: Violation:
Based on a record review of FC1?s file, the agency failed to provide documented evidence that FC1 had received dental care.

Findings:
1) In review of FC1?s record, with a placement date of 10-13-23, there was no evidence on file that FC1 had received dental care.
2) The Provider was given an opportunity to further search the record to locate evidence of any dental care arranged and FC1?s receipt of dental care.
3) Though the Provider stated that FC1 had received dental care and a recommendation was made by the dentist, the Provider was unable to locate documented evidence of dental care obtained for FC1 as of 2-13-24.
4) Agency representatives acknowledged the findings during the exit meeting on 2-20-24.

Plan of Correction: Plan of Correction:
Team will document any dental care received or any recommendations for dental treatment be signed by dentist or designee and note will be filed in client file.
Appointment was confirmed for 3/20/24.
On 2/27/24, Team received additional training on standard including the following:
E. Each child over three years of age shall have a dental examination. 1. The dental examination shall have been completed within 13 months prior to the time of placement or if no previous dental examination has been conducted, the child shall have an exam within 60 days following the date of placement. 2. The written report of examination shall contain the signature of a licensed dentist or his ---designee. 3. The licensee shall file a copy of the dental examination report in the child's file.
