Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 8/15/23 from 10:45 AM ? 2:54 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on 8/4/23 regarding allegations in the area(s) of: Buildings and Grounds, Medication and Nutrition. Number of residents in care at the beginning of the inspection: 5 Number of resident?s records reviewed: 1 Number of interviews conducted with residents: 1 Number of interviews conducted with staff: 4 Additional Comments/Discussion: An entrance meeting was held on 8/15/23 with the House Manager. The House Manager was accessible and available during the inspection. The following is a listing of the activities for this inspection: A resident?s record was reviewed. A medication administration record was reviewed. One resident was interviewed. Reviewed the facility?s policies and procedures in regard to medication. An inspection of the interior of the facility was completed. Four staff members were interviewed on-site at the facility. The Chief Executive Officer was interviewed by phone. The House Manager participated in the preliminary findings meeting held at the facility. An exit meeting was conducted on 9/20/23 to review the inspection findings. The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law.
