Comments:

An unannounced monitoring inspection was conducted on October 3, 2019 from 11:33 a.m. to 4:22 p.m. Two personnel records and two certified volunteer files, to include background investigation information, were reviewed. Six residents were in care. Four residents records were reviewed: three current residents' records and one discharged resident record. Other documentation reviewed at this inspection included but was not limited to the daily log, case management notes, emergency drill documentation, medications, and several medication administration records. Interior and exterior physical plant inspections were conducted. The Program Director was present and available to respond to questions during the inspection. One resident was interviewed. Exit meeting was held with the Program Director to discuss preliminary inspection findings before leaving the facility on October 3, 2019. An acknowledgement form was signed.
