Comments:

An unannounced monitoring inspection was conducted from 9:45 a.m. to 11:55 p.m. on February 20, 2020. Three personnel records were reviewed. Two residents were in care at the time of the inspection. Two current residents records were reviewed. The residents were not available to be interviewed as they were in school at the time of the inspection. Other documentation reviewed during this inspection included but was not limited to: the monthly resident evaluations, social work reports, counseling reports, emergency drill documentation, menus, and medical files Interior and exterior physical plant inspections were conducted. Staff S3 was present and available to respond to questions during the inspection. Preliminary inspection findings were discussed with Staff S3 before leaving the facility on February 20, 2020. An acknowledgement form was signed. An exit meeting was held with Staff S1 via telephone on February 24, 2020.
