Comments:

Type of inspection: Renewal Visit Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 4/16/2024 from 12:30PM ? 4:30PM 4/18/2024 from 2:55PM -4:30 PM The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of residents in care at the beginning of the inspection: 2 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident?s records reviewed: 2 Number of staff records reviewed: 3 Number of interviews conducted with staff: 2 Additional Comments/Discussion: An entrance conference was held on 4/16/2024 with the administrator/House Parent. The administrator was available and accessible both days of the inspection and exit meeting. The physical plant inspection and tour was provided for the administrative office and the home of the children. The preliminary findings meeting was held with the Administrator on 4/16/2024. Specialist and administrator discussed completion of background checks and requirement which are the same as an CRF for any new staff moving forward, utilizing the on-line background instructions that can be located on DSS web-page. An exit meeting was scheduled and conducted on 4/18/2024. Licensing inspector interview two sibling and inspection findings were reviewed with administrator. The evidence gathered during the inspection determined one violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary. Standard #:	22VAC40-100-350-E
Description:	Violation: Based on record review, the agency failed to maintain face sheets in the record with all the required components for child record 1(CR1) and child record (CR2). Findings: 1. CR1 and CR2 were both admitted 3/21/2011. 2. The record for CR1 and CR2 did not include a face sheet that addressed all required components per DSS standards. 3. The findings were discussed during the preliminary finding review and exit meeting.
