Technical Assistance:

The facility presented a revised policy and procedure manual. Licensing Inspector requested manual be provided by e-mail for review of compliance with Standards of Licensed Children's Residential Facilities.

Comments:

An unannounced monitoring inspection was conducted on October 29, 2019 from 11:10 AM - 4:42 PM and on October 30, 2019 from 9:35 AM to 12:14 PM. The facility reports a total of thirteen (13) residents in placement. During this inspection, records were reviewed for two current residents, two discharged residents, and three personnel. Staff were reviewed against the CRF Employee Matrix. Interviews were conducted with two children and three staff. A tour of the facility?s exterior and all living areas within the facility was conducted. Residential environment requirements were assessed. The following was reviewed for the past six months: daily logs, menus, staff work schedules, serious incident reports, emergency drill documentation, medication administration records for two residents, allowable variances, fire and sanitation inspection documentation, overnight trip documentation, and readily accessible medical information for two residents. An exit interview was conducted at the conclusion of the inspection with the Executive Director and Program Director to review preliminary inspection findings. An acknowledgement form was signed. There were no violations. The licensee has five (5) calendar days from receipt to sign each page of the inspection documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation will be posted on the VDSS web page and available for review by the general public.
