Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on September 18, 2020 and concluded on October 21, 2020. A complaint incident was received by the department regarding allegations in the areas of programs and services. The Executive Director was contacted by telephone to conduct the investigation. The inspector provided the Director a list of documentation required to complete the investigation. There were seven residents in placement. The evidence gathered during the investigation did not support the allegation of non-compliance with standards or law. The licensee has five (5) calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation will be posted on the VDSS web page and available for review by the general public.
