Comments:

An unannounced renewal inspection was conducted on April 7, 2022 from 10:52 a.m.to 3:27 p.m. Three (3) personnel records, to include background investigation information, were reviewed. Three (3) residents were in care. Four (4) residents records were reviewed (two current and two discharged). Other documentation reviewed at this inspection included but was not limited to daily log notes, emergency drill documentation, and medication administration records. Interior and exterior physical plant inspections were conducted. Administrator A1 responded to questions during the inspection. An exit meeting was conducted with A1 on April 7, 2022 where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Upon the receipt of the violation notice, the licensee should identify the necessary corrective action and develop a plan of correction for each violation. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Standard #:	22VAC40-151-210-B-1
Description:	Violation: Based on review of the personnel record for Staff S3 and interview with Administrator A1, S3 did not meet the qualifications required by the regulations for Child Care Worker. Findings: 1. Review of S3's resume did not reveal six months experience working with children. 2. A1 confirmed that the resume did not document this experience.
Plan of Correction:	Program Director will ensure that there is a qualified DSP staff available for shifts when a person is hired with less than 6 months of experience working with children.

Description:	Violation: Based on review of the personnel records for Staff 2 and Staff S3, written references were not included in the personnel records. Findings: 1. Written references or notations of oral references were not included in the personnel records for Staff S1 and S3. 2. A1 acknowledged that the personnel records did not include written references or notations of oral references.
Plan of Correction:	All personnel records will be scanned into each personnel?s file by the Chief Executive Officer or Program Director within 48 hours of receiving the documentation.

Description:	Violation: Based on review of the resident's record for discharged resident R3 and interview with administrator A1, the required information was not added to the resident's face sheet at the time of discharge. Findings: 1. The face sheet did not document the name and address of persons to whom the resident was discharged. 2. R3 discharged on 03/02/2022. 3. A1 confirmed that the name and address of the persons to whom the resident was discharged was not documented on the face sheet.
Plan of Correction:	Program Director or Case Manager will ensure that the face sheet will list the parent?s or legal guardian?s name and address as the person to whom the resident is being discharged.

Description:	Violation: Based on review of the resident's record for discharged resident R3, the discharge summary did not document the names of persons to whom the resident was discharged. Findings: 1. The discharge summary for R3 did not document the names of persons to whom the resident was discharged. 2. A1 acknowledged that the discharge summary did not document the names of persons to whom the resident was discharged.
Plan of Correction:	Program Director or Case Manager will ensure that the face sheet will list the parent?s or legal guardian?s name and address as the person to whom the resident is being discharged.
Standard #:	22VAC40-151-700-B
Description:	Violation: Based on review of the resident's record for resident R1 and interview with administrator A1, case management services were not documented in the resident's record. Findings: 1. Case management services were not documented in R1's record. 2. A1 confirmed that the case manager had not documented the case management services provided to the resident.
Plan of Correction:	Case Manager or Program Director will be required to meet with all new residents within 48 hours of their placements. The Case Manager or Program Director will scan all notes taken during each session with clients once the session is complete or no later than 24 hours to the resident?s electronic file.
