Comments:

An onsite monitoring inspection was conducted on 4/21/2022 from 1:00 p.m. to 5:00 p.m. An entrance conference was conducted with the Chief Administrative Officer/Program Director, Case Manager, and Human Resources & Data Manager. A tour of the facility and residential environment inspection was completed with the Chief Administrative Officer/Program Director. The facility reported one child in placement and no discharges. One resident record and 14 personnel records were reviewed. Daily logs, menus, Employee Matrix, and Structured Program of Care were inspected. Four staff and one resident were interviewed. An exit interview was conducted with the Chief Administrative Officer/Program Director and the Human Resources & Data Manager. Information gathered during the inspection determined no non-compliance with applicable standards or law, and violations.
