Type of inspection: Monitoring Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 12/28/2022 from 11:30 a.m. to 12:00 p.m. at the licensed location, 12/28/2022 from 12:25 p.m. to 1:30 p.m. at the administrative office, 12/29/2022 from 1:00 p.m. to 1:30 p.m. interview via Teams Meeting. The Acknowledgement of Inspection form was signed and left following the tour of the physical location. Number of residents in care at the beginning of the inspection: 1 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident?s records reviewed: 1 Number of staff records reviewed: 8 Number of interviews conducted with residents: 1 Number of interviews conducted with staff: 1 Observations by licensing inspector: The physical location suffered severe damage on 12/24/2022 due to a winter storm and the building was evacuated immediately. The facility implemented it?s policies and procedures for the emergency and is providing temporary alternative living at a local hotel. The facility continues to provide 24/7 awake supervision for the resident in care. A preliminary findings review was conducted at 2:00 p.m. 12/30/2022 by Team Meeting. Inspection documents were drafted and shared with the agency and an exit interviewed followed for final review. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary.
