Type of inspection: Renewal An unannounced, mandated renewal inspection was held on 7/20/23 from 10:50 a.m. to 4:30 p.m. A preliminary findings review was held on the same date. The Acknowledgement of Inspection form was provided via e-mail. An exit interview was held on 7/31/2023. Number of residents in care at the beginning of the inspection: 12 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility.
