Type of inspection: Renewal An unannounced, mandated renewal inspection was held on 04/18/2024 from 10:15 a.m. to 12:55 p.m. and 1:30 p.m. to 5:15 p.m. A preliminary findings review and exit were held on the same date. The Acknowledgement of Inspection form was signed and a copy left at the facility. Number of residents in care: 9 The licensing inspector completed a tour of the physical plant that included the residential buildings and grounds of the facility. Number of resident?s records reviewed: 4 Number of staff records reviewed: 8 Number of interviews conducted with residents: 2 Number of interviews conducted with staff: 3 Additional Comments/Discussion: The evidence gathered during the inspection determined no violations with applicable standard(s) or law.
